Nomination Form - High School Teacher of the Year Award

Greetings,

Each school year the Illinois Section of the American Association of Physics Teachers is pleased to receive nominations for the Illinois Outstanding High School Physics Teacher Award. This award is presented during the banquet at the spring meeting of the organization. It includes the citation, a plaque and a check for $100.

Fellow teachers and school administrators who are aware of exceptional performance and enthusiastic student response are encouraged to fill out this form to nominate this teacher. The deadline for receiving nominations is early December.

Following your nomination, this teacher will be contacted and asked to fill out the online "Candidate Information Form".
